Fire and Explosion Caused by Human Error and Equipment Failure

I’ve spent much of my 40+ year career in the petroleum and chemical industries. One might say I’ve pretty much seen it all when it comes to accidents. Unfortunately, I’ve have firsthand experience with the loss of good friends and coworkers due to what I call the human factor.

The human factor is at work every second of every day in every type of business and personal setting. It’s when hard working, good people get careless while performing routine everyday tasks. For whatever reason, a procedure intended to protect personnel is intentionally bypassed or carelessly forgotten and in most instances it involves only one or two steps being bypassed or forgotten that allows all of the stars to line up, and then tragedy strikes without warning which in most cases lead to serious injury and sometimes death.

I’ve spent much of my career using technology to create iron clad solutions that overcome human error and equipment failure in hazardous types of operations found in every day businesses that we are all familiar with. One such business is the loading of fuel onto large tanker trucks that take fuel to the local gas station that all of us pump into our cars. One such iron clad solution developed by Frontline Environmental addresses the more than 1500 spills that happen every day throughout the United States in truck, marine and aviation fuel loading facilities. The GuardDog™ is a patented technology designed to overcome human error and equipment failure during the loading of fuel at high transfer rates of more than 10 gallons per second. In “real-time” GuardDog™ continuously monitors the fuel delivery equipment to determine if it is performing properly or not. If not, the GuardDog™ will take a preemptive action to prevent the spill accident within 750 milliseconds. The Department of Defense and the USAF determined GuardDog™ to be 100% effective in eliminating spills over a 12 month period during a case study and real world test conducted at Travis Air Force Base. Due to its success Travis AFB has since standardized on GuardDog™ and is protected 24/7 365 days a year from spills.

Calibration Of Electrical Test Equipment

Maintaining and ensuring your test equipment is properly calibrated is essential if you are to obtain good and usable results from your experiments. Test equipment machines are only as good as the results they provide and if your machine is poorly maintained and inaccurate then obviously so will your conclusions be.

When you first purchase a brand new piece of testing equipment it should normally arrive with a certificate confirming that it was calibrated before leaving the manufacturers. Throughout the life of the apparatus it is important to have it regularly serviced and maintained and most importantly calibrated. The certificate that comes with re-calibration is your proof of a well-maintained machine and this will work in your favour when qualifying your test results.

A calibration certificate is also useful should you wish to sell any piece of machinery. You are more likely to get a good price if you are able to provide an audit trail, which proves that your equipment has been well looked after and cared for, and that its reliability can be confirmed.

Along with proof of calibration for quality audits and proof that your apparatus meets nationally accepted requirements it will also show the level and professionalism to which it has been calibrated.

When checking on the calibration certificate for a particular machine, especially if you are buying a used or second-hand model, make sure it has the following information: The correct instrument identification number, environmental conditions at the time of calibration, proof that the tests were conducted to a high standard and that there is an auditable, paper trail for the particular machine.

Quite often used models are sold as seen and you will have to pay an additional cost for calibration to be undertaken. Should you require this additional service, make sure you have the correct documentation afterwards.

You can also request a calibration certificate with data. While most certificates confirm the working parameters you may wish to have a copy of the actual data modes and ranges that the machine was tested against. Although tests conducted with data provided give you an accurate indication of the machine's capabilities, the extra information does also tend to cost that little bit extra.

Remember, whether you are buying a brand new test appliance or a second-hand one, or if you are perhaps leasing it, always check to make sure the equipment has been accurately calibrated to produce the desired information you are looking for.

As an engineer, Alex has re-calibrated used test equipment for years. Although his expertise is centred around electrical test equipment, he has been involved with project of many other natures.

Business Feasibility Study

A business feasibility study is an important primer for any entrepreneurial venture as it gives entrepreneurs insight to how well they can pull things off. As aspiring entrepreneurs, we must all be familiar with this form of research as it will become an integral part of our business lives. Feasibility studies have long been applied in order to prepare for a particular endeavor. And despite the advancements in technology we experience, nothing still beats the genius behind being able to see the future (or at least, the first few years of it).

A business feasibility study stands as the evaluation of a business proposal done to verify how difficult it will be to pursue a particular activity. It attempts to measure the complexity of the task in terms of financial and time expenditure as well as give a good estimate of the other resources that needed in order to implement the plans. Basically, what it aims at is to present a good analysis of the project in mind and its impact to society.

There are five common forms of feasibility studies. These are as follows:

1. Technology and system feasibility
This part of the study focuses on assessing if the system requirements of a certain business endeavor are possible - from input to processes, output, fields, programs and procedures. Researchers use different means to quantify if the business can handle the demand as well as see through the completion of the project using particular hardware, software, personnel and expertise.

2. Economic feasibility
This is the commonly applied feasibility analysis for business - whether pertaining to the actual establishment or just the pursuit of a new system. It is also known as the cost/benefit analysis and is directed at estimating the development and operational costs of the task at hand. It tries to compare the costs and the advantages/savings that are expected from the endeavor and determines whether the benefits outweigh the expenditures.

3. Legal Feasibility
This kind of feasibility study is aimed at finding out whether any aspect of the business plan goes against the judicial system or has legal requirements that need to be met. For example, aside from the permits, there are certain set safety standards regarding opening a new sports facility or amusement park. And all these need to be complied with in order to be fully operational.

4. Operational feasibility
Researchers and aspiring entrepreneurs should perform this type of study if they want to know how well a certain business endeavor addresses problems and meets the expected results determined from an early development phase analysis conducted.

5. Schedule feasibility
This kind of business feasibility study simply tries to foresee how long it will take for a project to be completed. It also tries to see whether or not changes can be made on the completion period if certain interventions were applied. Time is measured as it determines how reasonable a certain task is.
